This page was created specifically for our referring physicians in order to improve our communication and coordination of patient care.
You can download a referral form here for your convenience. You can also download our registration paperwork for your patients.
To better serve you and your patients, we accept walk-in appointments every day, Monday through Friday, from 8:00 am to 11:00 am and from 1:00 pm to 4:00 pm.
HPT’s ergonomic specialists will come to your office and do a complimentary ergonomic assessment.
- We provide quick and convenient scheduling—most patients are seen within two days.
- We provide a well-trained staff, who continue to gain professional skills and knowledge by attending accredited continuing education courses throughout the year.
- We provide you with timely, informative feedback regarding the patient’s care.
- We believe that communication between the referring physician, therapist and patient is key to successful rehabilitation.
- We welcome you to visit our facility at any time and talk to our therapists. We also would like to visit with you and your staff.
- We are open from 7:00 AM until 7:00 PM on Tuesdays and Thursdays for patients that must come in before or after work.
- HPT’s fitness specialists will assist in designing a wellness program for your office staff.
- We do ergonomic assessments in the workplace if you have a question about a patient’s ability to return to their job.
